A condition almost nobody talks about
Most of my hyperhidrosis clients have never raised it with a GP, a friend, or a partner. They've spent years strategising around it — choosing dark or patterned tops, avoiding lifting arms in photos, keeping a change of clothes at work, declining presentations or events.
Hyperhidrosis is more common than people realise — it affects an estimated 1–3% of UK adults — and the available treatment is remarkably effective, but most sufferers don't know it exists.
What hyperhidrosis actually is
Hyperhidrosis is excessive sweating beyond what the body needs for temperature regulation. Primary hyperhidrosis usually affects specific areas (most commonly underarms, palms, soles, or face) and tends to start in adolescence or early adulthood. It is not a sign of being unfit, anxious, or unwell — it is overactivity of the sympathetic nerves that signal sweat glands.
Antiperspirants help some sufferers but not all. Stronger prescription topicals (like aluminium chloride) can irritate. Oral medications (anticholinergics) have side effects that many find intolerable.
The treatment that actually works: Botox
Botulinum toxin — the same substance used for anti-wrinkle treatment — blocks the nerve signals that activate sweat glands. Injected just under the skin in the affected area, it produces a clear reduction in sweating within four to seven days, with full effect at around two weeks.
For axillary (underarm) hyperhidrosis, results typically last six to nine months. Some clients need annual treatment; others can stretch to 12 months.
What treatment is like
I apply a topical anaesthetic first. Treatment itself takes around 20 minutes. The injections are shallow and tolerated very well — most clients describe it as significantly more comfortable than they expected.
You can return to normal activity immediately. The first noticeable reduction in sweating is typically around day four; full effect by two weeks.
